$25 DEPOSIT is needed to make an appointment for any new patient.
If you are an existing patient with a set follow up appointment and you will need to pay a $25 deposit to reschedule.
There is no grace time for any appointments, this is an appointment based clinic.
Once an appointment has been made, there is NO REFUND on any deposit if you decide to CANCEL, NO SHOW, or RE-SCHEDULE.
Make sure to read all requirements for any treatment before booking an appointment, because appointment deposit will not be refunded in the case that you are not a candidate for the treatment.
We own the right to refuse services to anyone.
We have a zero tolerance policy for any bullying, harassing, or violent behaviors within patients or employees, we will discontinue services ASAP if we know there is any of these behaviors.
